Am I being impatient?

I've been on glps since 2022, started with sema, prescribed by a dr and lost 50lbs in the first 1.5 years. Then got kicked off by insurance and went grey. Switched for tirz after reading about the benefits and wanting to lose another 10lbs. Switched and it took me about another year to lose, since my weight was already on the lower end 135 to125 at 5'4 F. I got to the highest dose of 15mg, but it was wearing on me with side effects. I lowered my dose to 10mg and started gaining weight. This was back in April and my weight went up to 130, doesn't seem like a lot but 5lbs on smaller frame is noticeable. I decided that maybe my body needed a rest and I made peace with extra 5lbs. But then it kept creeping and between life situations and just being hungrier I was back up to 135. I decided to switch to Reta 7 weeks ago- started with 2mg for two weeks, then 4mg for 3 weeks and 6mg for weeks and this week I went up to 7mg. Zero weight loss... I exercise (walking, lifting weights, large, yoga and versa climber) and while Reta does seem to make me hungrier in some ways, I do have my diet dialed, following the 80/20 rule. Am I being impatient?? lolI

I ordered Cag and wanted to see if I should add or raise my reta dose? All feedback is appreciate.
 
At 135 your BMI is like 23, perfectly "good" level. Perhaps your body just doesn't want to be lower, there really is such a thing as a set point and while GLPs can help you get to a lower set point if you have a really high one, it can't do that infinitely.
 
I agree with the above and also add: generally people on max dose GLPs who take a break and then come back are going to need a high dose again.

high doses of reta somewhat intimidate me with the elevated heart rate I currently have that I immediately felt on 2mg.- now it has stayed same higher HR.
 
If you are on your meds and exercise, diet is what's left. Do you log calories? Weigh foods? Count everything that goes into your mouth? CICO is still a thing. Do you know that you are on a consistent calorie deficit?

Why didn't you tried switching back to tirz, if you had more success with it? So many success stories with Reta doesn't mean everybody will get the same results.

I was also in a good progres with Tirz and just out of curiosity and unconsciously willing to get faster results switched to Reta with no success. It has ended after 2 weeks, with no weight loss, getting hunger noise back and 3 days long heavy diarrhea. Switched back to Tirz and everything was back to normal within a few days. I don't claim that reta is not good or useless. But it was not good for me.

Results vary from person to person. Tirz is much better for hunger noise and hunger suppression. That is for sure. But causes muscle weight loss without active training and sufficient protein intake.
 
You are already at a perfectly healthy weight. If you want to weigh less, you need to use more calories than your body needs to maintain weight. You can do this the easier way by reducing calories or the harder way by burning more. Either way, the answer is calories not meds

Funny enough, this the first time on my glp 'journey' that I have started tracking calories/ weighing food. Started tracking when I started Reta 7 weeks ago. I'm averaging 1400-1700 cals a day depending on my physical activity. Food scale has made an appearance, who hasn't been used since prior to glp. I probably was consuming less previously because I just wasn't hungry.

Download the lose it app, set to sedentary, don't eat your exercise calories.

The reality is that at 46, perimenopausal, and 5 ft 6, my tdee is approximately 1600. I don't know how old you are but you are shorter and lighter than I am. Your tdee may very well be less than 1700. That absolutely does not mean you will gain weight at 1700 calories, but if you're looking to lose and not maintain you probably need to figure out your tdee and eat below that every day until you are at a weight you want to be at. Which really should not be much lighter than you are right now

I don't disagree, but as mentioned, my diet is dialed in. Now ( and a big now), I just wasn't hungry before on 15mg and was definitely eating less and not tracking. That then wore off and switched to Reta, which suppression wise is mid and I'm mainly asking if I should keep increasing since I'm only on 7mg reta and/ or add Cagrilintide. I'd imagine I have a pretty high tolerance since I've been on these meds for 4 years.

I'm 36. I have used TDEE cal and my cals are 1595 sedentary- I am far from sedentary. I just redid it again and I'd have to drop my cals to 1295. Which at this 7mg reta, I would be hungry lol.... I'd imagine uping my dose would help with the hunger part.

Pretty much how I do it. Except, I set my calories at 1500 and don't pay attention to the extra calories burned in activity. Some days I'm under 1500 and some days I'm above. Not by much either direction. Some days I'm less active and some days more. It all averages out in the end.
 
We need more info. Female, 36, 130ish lbs. Height? Because if you are 5'2'' reasonable to loose a little, 5'9'' possibly in eating disorder territory, unless like bikini competitor prep.

After helping a lot of people out in real life, most people don't track their food or track it well enough. Eyeball and underestimate, etc. I've had them take pictures of their meals so I can look at their myfitness pal log, and there has been some stark differences between the pic and whats logged.

If you are eating at a relatively small amount of calories, probably best to up them by a couple hundred and spend the next 3ish months trying to get stronger in the gym and let the weight go up a couple lbs. You will be stronger, likely rebuilt some lost lean mass and metabolism will be primed to lose. Sounds like you've been in a deficit for the last 4ish years?

Have you had your hormones checked like thyroid, estrogen, testosterone, IGF-1?

Also amazed you aren't starving on 7mg Reta coming off of 10-15mg tirz. Generally Tirz is more potent mg for mg.
 
When you are obese, the weight comes off ‘easier’ and this was something I heard yesterday…Idr who said it, but it wasn’t meant to mean weight loss is easy, just that small changes for obese people make larger noticeable results more quickly. At your lower body weight, and the closer to goal you get the more meticulous you have to be about everything.

You at 135, you may have to be patient and understand your loss will be slow. Cag maybe helpful…but only 7 weeks on Reta and quick dose escalation…it may not be fully in your system yet…I’d give it some time.

I do function health testing twice a year and I add hormone testing. So they’re in range- but who knows if that’s the right range for me.

I’d say the last few months I have been eating more and as mentioned to another commenter, I’ve actually been tracking my calories past 9ish weeks, right before I decided to switch from T to Reta. In September I weighed 125, and I’ve slowly gained 10lbs give or take- menstrual cycle makes 135 solid rest of month 132/133. Doesn’t seem like a lot but at 5’4 it’s not enjoyable. (I understand it’s trivial… wahhhh wahh)

What I really wanted advice on my post is, since I was on 15mg tirz should I keep at with Reta and raise my dose? Or add cag into the mix? It’s a challenge to adhere to this calorie drop on 7mg Reta. I understand cals matter but isn’t the point of the meds to be a tool to be able to adhere to the calorie deficit.

I appreciate the reality check and the last paragraph especially. I’ll keep the course and chill a bit. I’ll give it a few more weeks on Reta and then decide about adding cag. I’ll see how it goes.
