Just curious to know whether it is common experience to find resistance to RETA after being on TIRZ for a year. Had good results with TIRZ low doses and personally switched to RT. Am escalating the dose gradually but the results are not as per expectations. Have exercise , protein on board. Although I am just now on only 4 mg( a 7 month climb) .I read a lot of us are achieving much more fat loss with still lower doses . Recently stacked up with Mot C and TESA to hit the visceral component Would appreciate any pointers and suggestions?

Yes. If you've been on any glp before, you will need to be at higher doses to feel effects from another glp, or even the same glp if you stop and start again.

It all depends your health (obesity) starting point and how much weight you want to lose. Clinical data is clear that 20%+ weight loss is only acheived at clinical doses of 8-12mg, not less. Tesa is good at visceral fat but doesnt do as well for overall weight loss. Stick with reta for that. A 7 month climb to 4mg is also exceedlingly slow. Ramp it up! Youll see results.

Cheers!

Thanks for the input. I started TZ at 135 kg , got it down to 110 , shifted to RT but have seen a maximum of 15 kg loss in 7 months time. I will ramp it up at 14 day intervals by 0.5 mg /1mg ? . Want to continue the Tesa for the next 3 months . Still got to lose another 20 kg .. I hope to get there by december ..

What was your tirz dose? You mentioned that you had good results on low-dose tirz and lost 25kg, but then switched to reta. I wonder whether you could have just continued titrating up on tirz if you were still only on a low dose.

As for tesa, I personally would have added it later, when you’re closer to your goal weight. I’m not sure how much of a difference you’ll notice while you still have 20kg to lose. Not that it wouldn’t help, but there’s already a lot going on right now with transitioning from tirz to reta, adding MOTS-c, etc.

You’re also losing just over 2kg a month (15kg in 7months), which is decent progress. Weight loss will naturally slow as you get leaner, so I’m not sure the answer is to keep ramping up reta every two weeks in an attempt to lose the remaining 20kg in five months (4kg per month).

Standard ramp up for reta is 2/4/8/10/12mg ramping up dosage every 3 weeks or so. So 0.5mg every two weeks is slow, 1mg is right around there but slow as well.

You want to loose 20kg in 4 months. Thats about 5kg per month. You will need to ramp up dosage way higher and faster imo. 8mg is a sweet spot for reta so I’d try to get there at the very least. You want to loose another 15% of your initial bodyweight after having lost a significant amount. 30%+ is only really probable on 12mg of reta on a signficant amount of the population. That isnt to say you cant get there on 8mg but just less probable.

Like others said, i would only add tesa at the end once you’ve acheived your weight goal or close to it. It doesnt lead to signficant weight loss and its just really a tool to help you loose stubborn visceralfat.

Hope this helps!

Thanks for your input, I was on 5 mg TZ, but lost all interest in food and it became a chore, I also felt i could not get the weight down any more with that dose , took a 2 month break and then switched to RT.

Thanks I will ramp up 2 mg monthly and see how my body handles it , some people are doing really well with low doses , maybe I am not one of them . I have upped my game on increasing weights and protein intake , lets see where it goes ..

Thanks , I will ramp up my weekly dose and see where it goes I guess i haven't hit my sweet spot yet. With TZ even 5mg was a lot , that doesn't seem to be the case with RT yet..

If you are trying to lose a total of 45kg from a start at 135kg, that is about 33%, so quite a lot and higher than the average best results from the reta trial, but so far you seem to be doing pretty well from fairly low doses, so you might be more sensitive than average. My guess is in the long term to get to 33% down or close to it you are very likely to end up requiring higher doses and most likely maximum doses of 12mg of reta, assuming side effects allow this. Does not mean you need to increase doses faster, but it is very likely you will need to go higher eventually.

Is achieving more fat loss on a lower dose an indication being somewhat better than the other person? Your dose is your dose, don't compare yourself to other people, and increase your dose. Lose the tesamorelin, you will not like seeing the scale go up while toy put in water weight on it.

HI Thanks for your insights , I agree with you , as a physician myself the pharma rep was pushing me to dose up ( while on TIRZ) but having hit side effects on 5mg I did not ramp it up. Took a 2 month break and then decided to give RT a shot. I see a lot of my diabetic patients losing good amount of their fat on TZ over 2/3 years . Everyone has a different journey.

RT has been slow and steady. I probably have not hit my sweet spot. Time is really not an issue. Metabolic profile is already looking good while on RT. I will ramp up and see how I do on it. I hate to see skin hang and probably that is why i took the gradual climb.Hope to get there ..

Hi , No not really it does't have anything to do about comparing. Every individual has a different physical and metabolic profile. WIth RE`TA I probably need more than what I am on currently. I am 2 weeks into TESA will evaluate my measurements at 1 month and see whether to ride it out for 8 weeks or stop it. The journey continues ..
